AN INQUEST opened into death of a Rhyl teacher.
Vivienne Greener, of Lilac Avenue, died at Glan Clwyd Hospital on March 20.
A post mortem gave the provisional cause of death as multi-organ failure due to sepsis.
During the brief hearing, held at the County Hall in Ruthin, it was heard that Mrs Greener had been formally identified by her husband.
John Gittins, coroner for North Wales East and Central, said some concerns had been expressed.
Betsi Cadwaladr University Health Board (BCUHB) also conducted an investigation.
The inquest will be listed shortly - no date has been set as of yet.
